The Ministerial credentials Board meets yearly with new and returning candidates to review the status of their application. The forms provided below are required by every candidate. All candidates for a Minister's License must have a Recommendation Form filed out but the churches local church board. This form needs to be turned into the Ministerial Credentials board prior to the yearly meeting.
